This wild-looking little thing completely puzzled me. At first I was thinking beetle or month larva, until I got a look at the pictures on the computer screen. I made a couple of incorrect guesses before entomologist Greg Courtney pointed me in the right direction with Psychodidae. He suggested a possible genus of Thornburghiella, but could not rule out some other members of the tribe Pericomini.

5 Example Specimens
Key characteristics verified under the microscope:
-The head has 3 ocelli. -Maxillary palp has 5 segments, with the second segment rounded. In some of the pictures it looks like 6 with the 3rd segment rounded, but the basal "segment" in that case isn't part of the palp. -Foretibiae lack preapical spurs. -Pronotum with mesal setose warts widely spaced. All of this very clearly indicates Glossosomatidae. Beyond there: -The tibial spur counts (fore / middle / hind tibiae) are 2, 4, and 4. -From the pictures, the hind wing discoidal cell is closed, not open. Subfamily Glossosomatinae. -The upper and lower parts of the mesepisternum are separated by a constriction, not a transverse suture. Microscope picture included. This leads to a very confident genus ID of Glossosoma. I don't have the information to take it to species. 
I'm tentatively classifying this one as Dibusa angata because I cannot see any ocelli in my dorsal view of the head (and because it has a single spur on each front tibia). However, sometimes they can be difficult to spot when obscured behind setae in general, so I'm not 100 % sure on this one. If there are ocelli I missed, then based on ranges and colors the genus ought to be Stactobiella.
I lost track of this specimen before I could get it under my microscope, but caddis expert Dave Ruiter was able to identify it from pictures as Glossosoma, with an uncertain suggestion of G. alascense as the most likely species.
